What is the real difference?

A restroom trailer is a towed bathroom with porcelain flush toilets, running water, lights and climate control, and it needs power. A porta potty is a single portable stall; a deluxe porta potty adds a foot-pump flush and a sink but needs no power. Trailers feel like a building; deluxe units feel like an upgrade.
